ABSTRACT

BACKGROUND: For autologous hematopoietic stem cell transplantation (HSCT), the volume of infused and DMSO contained in graft are the major causes of complications related to infusion. In this study, we evaluated feasibility of cryopreserving peripheral blood stem cell collects (PBSCC) at high cell concentration. METHODS: PBSCC from 40 patients with multiple myeloma or lymphoma were split and cryopreserved at two different concentrations of TNCs; one for standard concentration (SC) (2x108 cells/mL) and the other for high concentration (HC) (3x108 cells/mL). The viability of total nucleated cells and CD34+ cell count were examined before cryopreservation and after thawing. CFU-GM was examined with thawed products. Data were analyzed as two groups between good mobilizer (GM) and poor mobilizer (PM). RESULTS: There were no differences in TNC viability between SC and HC of all patients (P=0.0656) and PM (P=0.9658), however HC of GM showed significantly lower viability than SC (P=0.0314). CD34+ cell viability did not differ between SC and HC. CD34+ cell recovery was decreased in HC of all patients (P=0.459) and GM (P=0.0164), but no differences between SC and HC in PM (P=0.9658). CFU-GM clonogenic efficiency between SC and HC was not different in all patients (P=0.0635) and PM (P=0.8984), but was decreased in HC of GM (P=0.0427). CONCLUSION: Cryopreservation of PBSCC at 3x108 cells/mL seems to have minimal adverse effect on the quality of PBSC after thawing, particularly in PM. This approach may help to reduce infusion related complications while decreasing the cost of processing and storage of PBSCC.

ABSTRACT

PURPOSE: This study analyzed the tumor response, overall survival, progression free survival and related prognostic factors in patients with muscle invasive bladder cancer subjected to bladder preserving treatment. MATERIALS AND METHODS: Between August 1995 and June 2004, 37 patients with muscle invasive (transitional cell carcinoma, clinically stage T2-4) bladder cancer were enrolled for the treatment protocol of bladder preservation. There were 33 males and 4 females, and the median age was 67 years (range 38~86 years). Transurethral resection of the bladder (TURB) was performed in 17 patients who underwent complete resection. The median radiation dose administered was 64.8 Gy (range 55.8~67 Gy). The survival rate was calculated by the Kaplan-Meier method. RESULTS: An evaluation of the response rate was determined by abdomen-pelvic CT and cystoscopy at three months after radiotherapy. A complete response was seen in 17 patients (46%). The survival rate at three years was 54.7%, with 54 months of median survival (range 3~91 months). During the study, 17 patients died and 13 patients had died from bladder cancer. The progression free survival rate at three years was 37.2%. There were 24 patients (64.9%) who had disease recurrence: 16 patients (43.2%) had local recurrence, 6 patients (16.2%) had a distant recurrence, and 2 patients (5.4%) had both a local and distant recurrence. The survival rate (p=0.0009) and progression free survival rates (p=0.001) were statistically significant when compared to the response rate after radiotherapy. CONCLUSION: The availability of complete TURB and appropriate chemoradiotherapy were important predictors for bladder preservation and survival.

ABSTRACT

Febrile nonhemolytic transfusion reaction (FNHTR) is one of the most common adverse reactions to the transfusion of blood products. It is caused by antigen-antibody reactions between patient's plasma and transfused cellular components (or the reverse) or bioactive substances such as cytokines generated by leukocytes during storage of cellular blood components. Most of the antibodies involved in FNHTR are anti-HLA antibodies. However, platelet-specific antibodies and rarely granulocyte-specific antibodies may be involved in FNHTR, We found a granulocyte antibody from a 53-year-old male with FNHTR for the first time in Korea. Fever developed during transfusion of a unit of packed RBC after partial cystectomy, and subsided after the adminstration of acetaminophen. The hemolytic transfusion reaction and sepsis were excluded after investigations. Mixed passive hemagglutination test revealed that the patient had antibody against human neutrophil antigen-1b (HNA-1b), and granulocyte antigen genotyping showed granulocyte antigen mismatches between the patient (HNA-1a homozygote) and the unit of transfused RBC (HNA-1a/HNA-1b heterozygote).

ABSTRACT

Traumatic dislocation of the testis is defined as an ectopically displaced testis as the result of trauma. It is usually associated with motorcycle accidents, but a few cases have been reported from incidents ranging from kicks to the perineum to falling accidents. Traumatic dislocation of the testis occurs as a result of direct pressure exerted on the scrotum causing the testicle to be expelled into the surrounding tissue. We experienced 3 varying cases of superficial inguinal dislocations of the testis. One was due to a motorcycle accident, another to kicks and one to a falling accident. Spontaneous reduction is rare, and early surgical reduction is often required.

ABSTRACT

Various intravesical foreign bodies have been extracted from the bladder. Their introduction into the bladder may be through self-insertion, iatrogenic insertion or migration from the adjacent organs. The means of extracting the foreign bodies differ according to the nature and characteristics of the foreign bodies and to minimize bladder and urethral injury. However, complete extraction should also be confirmed after the extraction procedure. Bladder stone formation in an intra-uterine contraceptive device that perforates into the bladder seldom occurs. We experienced a case of a bladder stone that formed in an intra-uterine contraceptive device, which had perforated into the bladder. The possibility of an intravesical foreign body should be considered in any patient with chronic unexplained lower urinary tract symptoms.

ABSTRACT

We report a case of clear cell acanthoma occurred in a 61 year old man. He developed a brownish bean sized nodule on the left chest 3 years ago. The skin lesion has slowly increased in size without any subjective symptoms. Histopathologic examination revealed proliferation of clear cells within a sharply demarcated area of the epidermis. The presence of numerous neutrophils throughout the epidermis were also noted.

ABSTRACT

We have performed three total wrist arthroplasty of Meuli type on the dominant wrist of three female patients who have been suffering from the classical rheumatoid arthritis for more than two years, and after follow them for average twenty months postoperatively, we have found, even the complications and reoperation rate after total wrist arthroplasty are reported high in literature, three patients have reasonably satisfied with the remarkable relief of pain and the functional abilities of their wrists. It is our belief, even if the total wrist arthroplasty is also still in controversy, that in those patients with bilateral wrist involvement by the rheumatoid arthritis in whom arthrodesis was done on one wrist, then a total wrist arthroplasty on dominant wrist is a most prefer way to reconstruct the disabled wrists.

ABSTRACT

Among 13 patients with pemphigus : 8 of pemphigus vulgaris(PV), 4 of pemphigus foliaceus(PF), l of pemphigus erythematosus(PE), we have examined the sites of early lesions and the clinical appearances of the skin lesions. The most common site of the early lesions in PV was the oral mucosa, however, in PF and PE the face was affected first. The clinical appearances of the early lesions were flaccid bullae, erythematous papules and eczematoid patches. Clinicians should be aware of these clinical features of pemphigus for a more precise diagnosis and better management.

ABSTRACT

We report a case of 64-year-old female patient who has had factitious skin lesions for 20 years. She complained formification sense and severe itching on her face. These symptoms were improved with squeezing and bleeding. There were scuare shaped ulcerated nodule on the right cheek and white depressed scar on the left cheek. She was treated with wet dressing and occlussive dressing, and skin lesions were nearly cleared within 3 weeks, but new lesion appeared. Pimozide 2mg #1 p.o were given under the diagnosis of delusion of parasitosis from June, 1984. Her symptoms were improved without recurrence till now. Treatment with pimozide in monosymptomatic hypochondriacal syndrome is discussed.

ABSTRACT

The second case of this serial reports under the title of unusual cutaneous manifestations of connective tissue diseases is a woman with primary idiopathic dermatomyositis who developed cutaneous ulcerations on her arms and legs, which is thought to b a rare manifestation of adult dermatomyositis. These multiple ulcerations are about bean sized, polymorphous, and grouped on the poikilodermatous lesional skin. In this patient these ulcerative lesions became regressive and healed with disfiguring scars after the activity of the disease has controlled by prednisolone and methotrexate.

ABSTRACT

Solar urticaria is a rare skin disease characterized by urticarial reactions immediately following exposure to sunlight or artificial radiation. A 24-yr-old man had solar urticaria that was activated by visible light. Positive result of passive transfer of the reactivity with the patients serum to the skin of normal recipient suggested that his condition was attributable to an allergic response. Result of reverse passive transfer studies was negative. We report a case of solar urticaria which appeares to belong to type N in the classification of Harber.

ABSTRACT

Recent studies indicated that cellular and humoral immune reactions against Afycobacterium leprae may be involved in the development of some of the pathological manifestations. In lepromatous leprosy one might anticipate the formation of immune complexes from concomitant occurrence of large amounts of mycobacteria and corresponding antibodies. Indeed, there are some evidences of an involvement of immune complexes in the pathogenesis of erythema nodosum leprosum(ENL), mainly based on clinical and pathological studies. Some of numerous immune cornplex tests have been applied for the study of leprosy, But no single test has yet been shown to be the rnost sensitive and the most specific for the entire spectrum of circulating immune complexes. Furthermore. Leprosy has been found to differ in its clinical manifestations and immunological behaviors in different geographic areas and races. The authors considered it is necessary to determine the circulating immune complexes in Korean patients with leprosy to evaluate the immune status and the differences in the immunologic reactivity between the lepromatous and the tuberculoid spectrum of Ieprosy.

Atopic dermatitis is one of the common dermatoses characterized by its chronicity with an uncertain prognosis and usually occurs in patients with a personal or familial history of atopy. There is a dearth of information on the incidence and clinical manifestation of atopic dermatitis in Korea. ABSTRACT

Senile sebaceous hyperplasia refers to a benign enlargement of a normal sebaceous unit which is not uncommon in the old age group, and is characterized clinically by single or multiple small cream to yellowish round papules developed on the face, chiefly on the forehead and cheeks. A case of florid senile sebaceous hyperplasia developed in a 81-year-old man was presented. Because this disorder is one of the geriatric dermatoses and the average life span of our people is remarkably lengthened in recent years, we believe dermatologists should pay more attention to this kind of dermatosis.

ABSTRACT

The close similarity of acrokeratosis verruciformis(AKV) to the acral lesions of Dariers disease is well known. However, the exact relationship between the two disorders has not been satisfactorily resolved. A case of Dariers disease associated with guttate leukoderma and AKV is presented. The patient was a 17-year-old male who had dark cursted papules on the neck and lower abdomen, flat-topped keratotic papules on the dorsal surface of hoth hands, and white macules disseminated on the trunk and lower extremities of 7 years duration. The histopathologic examination of the lesion on the lower abdomen revealed parakeratotic plugs, corps ronds, and suprabasal clefts. The lesion on the dorsum of the hand showed compact hyperkeratosis, acanthosis, and papillomatosis with the pattern of church spires. The leukodermic lesion revealed decreased melanin content in the epidermal cells.

ABSTRACT

The nipple involvement in atopic dermatitis is not mentioned in currently available major textbooks of dermatology. Recently, Hanifin and Rajka indicated that the nipple involvement is a specific indicator of atopic dermatitis though it is quite rare. The authors described 3 cases of atopic dermatitis with nipple involvement among 224 cases of atopic dermatitis seen over 2 year period at the Special Clinic for Atoipc Dermatitis. The nipple lesions responded to topical steroid quite satisfactorily.

ABSTRACT

The formation of spontaneous rosettes between human lymphocytes and sheep erythro ytes under suitable conditions is a constant feature of T-lymphocytes. Recently. It has been demonstrated in the mouse, rat, guinea pig and in man that a certain proportion of peripheral blood lymphocytes has the property to bind autolognus erythrocytes in vitro. These lymphocytes are called auiologous rosette forming cells(ARFC). Although there are several reports on human ARFC. the results are not in accordance. The present study was undertaken to demonstrate the autologous rosette forming property of human peripheral blood lymphocytes, and to enumerate the percentage of ARFC in different sex and age guoups. Fifty healthy persons entered this study between Martch and September. 1980. at the Department of Dermatolgy, Seoul National Universty Hospital. The method of quantitation of AREC was s sight modification of that of Lambermont et al(1977). The results were as follows.

ABSTRACT

It has been proposed that action spectrum in polymorphous light eruption lies predominantly in the sunburn range, but reaches through the long-wave ultraviolet (UV) into visible part of the spectrum. We examined the efficacy of UUB, UVA and visible light in reproducing skin lesions, and found that the UVA was most suitable in provoking the typical skin lesions in our 3 cases with polymorphous light eruption, Plewig and his associates reproduced the clinical lesions with megadoses of UVA,(60J/cm2); in our experiment, 5 to 10 J/cm2 was sufficient. The reason for this discrepancy is not clear but the method of phototest, light source and energy, the different susceptibility in different races and other factors may influence the phototest results.
